| Briefly Describe the Purpose of the Program | ||
| ICC provides technology and assistive devices to older adults living in the community, with the goal of enhancing their overall well-being and quality of life. To be eligible for this program, individuals must be 60 years of age or older and be referred by an approved referral source, such as an Area Agency on Aging (AAA), AAA-funded entity, Care Coordination Unit (CCU), or Adult Day Service (ADS) provider. Older adults may, but do not need to be, receiving aging services through their referring agency at the time of referral. Referrals are received and processed by the Illinois Assistive Technology Program (IATP), with whom IDoA partners to administer this program. Examples of available devices include: tablets, hotspots, keyboards, headphones, cane, walker, wheelchair, shower chair, adaptive kitchen tools and utensils, non-prescription vision and hearing devices, video doorbell, keyless smart lock, voice-activated home assistant, smart lightbulbs, and robotic pets. Devices and usage instructions are provided at no cost to recipients and are theirs to keep (with minimal exceptions for certain loaner devices). Within three months of delivery, IATP staff will contact the recipient to ensure they are satisfied with the product(s) they received. | ||
