Long-Term Care Ombudsman Program: Services are delivered through provider agencies and individuals designated by the Office of the Long-Term Care Ombudsman and are operated through a grant or contract with IDoA or an Area Agency on Aging (AAA). Home Care Ombudsman Program: Services are delivered through individuals designated as ombudsmen by the Office of the Home Care Ombudsman.

Briefly Describe the Purpose of the Program
Long-Term Care Ombudsman Program: The Illinois Long-Term Care Ombudsman Program advocates on behalf of residents and their families in long-term care facilities. The ombudsmen also intercede on behalf of individuals and groups to ensure that they receive the benefits and services to which they are entitled and that their rights guaranteed by law are protected and enforced.

Home Care Ombudsman Program: The Illinois Home Care Ombudsman Program works to resolve complaints made by or on behalf of persons living in the community who are receiving services through the Medicare Medicaid Alignment Initiative (MMAI) and older adults and persons with disabilities who receive services through select Home and Community Based Services Medicaid Waivers. The goal of the Illinois Home Care Ombudsman Program is to empower, advocate on behalf of, and protect the rights of individuals who live in the community and receive services to help them stay in their homes.

Eligibility Requirements or Restrictions
Long-Term Care Ombudsman Program: Provides services to persons 18 or older who are either a current resident, a prospective resident, or a former resident of a long-term care facility; friends and relatives of persons who live in long-term care facilities; long-term care facility staff members and administrators with resident-related concerns; individuals and families who are considering long-term care facility placement as a long-term care option; the community at large; and other interested groups concerned about the welfare of residents of long-term care facilities.

Home Care Ombudsman Program: Provides services to older adults and adults with disabilities living in the community who are enrolled in the Medicare Medicaid Alignment Initiative (MMAI) and receive services from a Medicaid Managed Care Organization (MCO); or receive services from one of the following Home & Community-Based Medicaid Waiver Programs: Persons Who are Elderly, Persons With Disabilities; Persons With Brain Injury, or Person with HIV or AIDS.
